What is Metformin?

Metformin is a prescription diabetes treatment approved by the United States Food and Drug Administration. The medication is only available with a doctor's prescription. This medicine is available in two forms i.e. an oral tablet form and an oral solution form. This drug works by lowering hepatic (liver) glucose synthesis, lowering GI glucose absorption, and increasing target cell insulin sensitivity.

This medicine is used to enhance glycemic (blood sugar) management in persons with type 2 diabetes as a supplement to diet, exercise, and lifestyle modifications such as weight loss. Metformin does not make you gain weight. This medication can be used as part of a multi-drug treatment. As a result, you may need to combine it with other medications.

There are two types of metformin oral tablets i.e. immediate-release type and extended-release type. Generic versions of both pill forms are also available. Generics are frequently less expensive than their brand-name counterparts. They may not be available in all strengths or forms as brand-name medications in some circumstances.

Uses of Metformin

Dosage for type 2 diabetes

Adult dosage (ages 18–79 years)

  • Immediate-release tablets
    • 500 mg twice a day or 850 mg once a day is a common starting dose. Take your pills with food.
    • Dosage changes:
      • Your doctor may increase your dose by 500 mg every week or 850 mg every two weeks, up to a total of 2,550 mg in divided doses every day.
      • If your doctor prescribes a dose of more than 2,000 mg per day, you may be required to take it three times each day.
      • Maximum dosage: 2,550 mg per day.
  • Extended-release tablets
    • Typical starting dosage: Take 500 mg with your evening meal once a day. Except for Fortamet, this applies to all ER pills. Fortamet is usually started with 500–1,000 mg taken once a day with your evening meal.
    • Dosage changes:
      • Every week, your doctor will increase your dose by 500 mg.
      • If once-day dosing fails to regulate your blood sugar, your doctor may divide your total daily amount and have you take it twice daily.
    • Maximum dosage: 2,000 mg per day. 

Child dosage (ages 10–17 years)

  • Immediate-release tablets
  • Extended-release tablets
    • This medication hasn’t been investigated in infants younger than 18 years of age.

Child dosage (ages 0–9 years)

  • 500 mg twice a day is a common starting dose.
  • Changes in dosage: Every week, your doctor will raise your dosage by 500 mg in divided doses.
  • 2,000 mg per day is the maximum dose.

This medicine hasn't been evaluated in children under the age of ten, thus it shouldn't be given to them.

For Adults With Type 2 Diabetes Mellitus

  • Monotherapy or with sulfonylurea
  • Immediate-release tablet or solution
    • Initial dose: 500 mg every 12 hours or 850 mg once a day with meals; increase every two weeks.
    • Maintenance doses: 1500-2550 mg/day, split orally once every 8-12 hours with a meal.
    • Do not exceed 2550 mg per day.

Before taking this medicine

  • If you have recently had or acquired any of the following problems, tell your doctor: major infection; severe diarrhea, vomiting, or fever; or if you drink much less fluid than normal for any reason. It's possible that you'll have to stop taking metformin until you feel better.
  • If you've ever had an unusual or adverse reaction to this or any other medicine, tell your doctor.
  • Tell your doctor if you're taking metformin if you're having surgery, including dental surgery or any other major medical operation.
  • If you consistently consume excessive amounts of alcohol in a short period of time, tell your doctor (binge drinking). Drinking alcohol raises your risk of developing lactic acidosis and lowers your blood sugar levels.
  • If you've ever had kidney disease, high ketone levels in your blood or urine, heart disease, congestive heart failure, liver disease, or if you're on insulin or other oral diabetes treatments, tell your doctor.
  • If you are pregnant or plan to become pregnant, follow your doctor's instructions for taking metformin.
  • Metformin may raise the risk of unplanned pregnancy in premenopausal women by stimulating ovulation. Discuss your risk with your doctor.

How to use Metformin?

  • Follow your doctor's instructions for taking metformin.
  • Follow your doctor's customized food plan to the letter. This is an essential aspect of managing your illness and is required for the treatment to operate effectively.
  • Also, exercise on a regular basis and test your blood or urine for sugar as advised.
  • Metformin should be taken with food to help minimize stomach or intestinal side effects that may develop during the first few weeks of treatment.
  • Metformin is available in the Indian market as a liquid, tablet, and also extended-release (long-acting) tablet for oral administration. One or two times a day, the liquid is taken with meals. The standard tablet is usually taken two or three times a day with meals. The extended-release tablet medicine is usually taken with dinner once a day. Take metformin at the same time(s) every day to help you remember to take it.
  • Metformin extended-release tablets should be swallowed whole, not divided, chewed, or crushed.
  • Carefully measure liquid medicine. Before measuring a dose, give the oral solution a good shake. 
  • After your body has absorbed the drug, a portion of the extended-release pill may travel into your stool. This is completely normal and nothing to be worried about regarding that.
  • Your doctor may start you on a low dose of metformin and progressively raise it once every 1–2 weeks or less.
  • You'll need to keep a close eye on your blood sugar levels so your doctor can assess how effectively metformin is working.
  • Metformin helps to manage diabetes, but it doesn't cure it. Even if you feel fine, keep taking metformin. Do not discontinue taking metformin without first consulting your physician.

If any of these symptoms are severe, do not go away, come back, or do not appear for a long time after you start taking metformin, tell your doctor:

  • low blood levels of vitamin B-12
  • unpleasant metallic taste in the mouth

Some side effects can be serious. Call your doctor or seek emergency treatment if you suffer any of these symptoms or those indicated in the IMPORTANT WARNING section:

  • chest pain
  • rash
Missed or Overdose of Metformin

Take the drug dosage as soon as you remember to take the dose, but if your next dose of medicine is approaching, avoid the missed dose. Do not combine two dosages of your medicine at once.

Severe hypoglycemia or lactic acidosis can result from an overdose. If the person has passed out, had a seizure, is having problems breathing, or cannot be woken, call your doctor immediately.

Symptoms of overdose may include hypoglycemia symptoms:

  • extreme tiredness
  • weakness
  • discomfort
  • vomiting
  • nausea
  • stomach pain
  • decreased appetite
  • deep, rapid breathing
  • shortness of breath
  • dizziness
  • lightheadedness
  • abnormally fast or slow heartbeat
  • flushing of the skin
  • muscle pain
  • feeling cold

Storage

  • Keep this medication between 68°F and 77°F (20°C and 25°C) at all times. It can be kept at temperatures between 59°F and 86°F (15°C and 30°C) for a short time.
  • Keep the drug away from heat, moisture, and direct light in a closed container at room temperature. Prevent the freezer from freezing.
  • This medication should not be kept in a moist or wet environment.
  • Keep out of children's reach.
